I took it upon myself to design a page for my log just have the coolest looking log in the gym. Well here the page from it. Yall are free to print it and use it if you like...

You got to have adobe acrobat reader... pretty much all computers have this program now a days...

yea just write your reps and weight used in the set boxes

Nice! I'm confused though, why do you have "exercise" on there twice? Is the "exercise" and "duration" mainly just for cardio and the rest for weights?
 
I have not used a log in some time now.
Yours looks nice a easy to work with.
Maybe its time to see where I am and where Im going...
